Station brief (8–10 minutes)
Counsel the young person and parents. Outline a structured transition plan. Do not invent local legal ages or benefit rates. [1]
Tasks for the candidate
- Greet the young person first and use accessible language. [17]
- Explain transition as a process, not a birthday. [1]
- List practical next steps: readiness, summary, emergency plan, adult providers, legal prep. [1] [7]
- Address parent anxiety with partnership language. [8]
- Safety-net: what to do if seizures worsen or adult clinic is not secured before transfer. [1]
Expected performance
Must hit. Process not dump; youth included; portable summary + emergency plan; named adult homes; parent preparation; unsafe to discharge without receiving care. [1] [7] [8]
Merit. Mentions readiness tools as teaching maps; warm handoff; post-transfer check. [3] [12]
Fail. Birthday discharge only; ignores youth; invents legal cut-offs; no emergency plan. [1]
Sample candidate structure

We will build skills and supports, write a clear summary and seizure plan, find adult doctors who accept you, prepare legal decision supports with local advice, and introduce those teams before we transfer.
