Candidate: Planned movement from child-centred to adult-oriented care with continuous services — preparation, transfer and integration — not notes alone. [1]
Branch B — Readiness
Examiner: TRAQ is low. Stop transfer? [3]
Candidate: No. Use the score as a curriculum and support plan; build skills and adult accommodations; do not abandon care. [3]
Branch C — Family and youth
Examiner: Parents are anxious; youth is minimally verbal. [8]
Candidate: Include the young person with communication supports; prepare parents with timeline and joint planning; avoid speaking only about the youth as if absent. [8] [17]
Branch D — Failure mode
Examiner: At 18 they are discharged with no adult clinic. [1]
Candidate: Unsafe. Escalate to secure accepting adult providers, complete emergency plan, and avoid dump transfer. Re-engage urgently if seizures or loss to follow-up. [1] [12]
